Your doctor may prescribe antibiotic ointments or steroid drops . In some cases, an injection of Triamcinolone (Kenalog) is recommended, but taking this drug is associated with small risks of steroid deposition on the skin, necrosis, atrophy of subcutaneous fat, loss of vision, incomplete resolution of the stye, and the need for surgery. Other adverse reactions include hypo- or hyperpigmentation, especially in patients with dark skin tones.
Surgical treatment of stye
If inflammation does not go away after taking medications, surgical incision and drainage . The operation is performed under local anesthesia: an anesthetic is injected into the affected eyelid through the skin, or the conjunctival surface, or through both areas.
During the operation, the eyelids are everted. A chalazion clamp is used to hold the eyelid in place at the center of the stye.
Drainage is performed using a stab incision with a special blade over the affected area. External incisions can cause scarring, so making cuts or punctures on the outside of the eyelid is less advisable. They are carried out if the stye is directed to the surface of the skin. In this case, the incision is made directly above the area of thinning skin.
Internal incisions can be made vertically along the length of the meibomian gland. This incision seals itself, so the cornea is less irritated as it heals. The contents from the lesion are removed with a small curette. To ensure the release of residual pus, the incision is left open with clean edges and drained, for example, with a piece of latex - this avoids the wound from healing until it is completely emptied. Then the eyelids are covered with ointment for several hours [7].
Symptoms of the disease
The first signs of stye are usually local redness on the eyelid, accompanied by itching and/or pain. The affected area quickly swells, taking on the characteristic appearance of a mound with a yellow center and remaining painful.
There may be a fairly significant increase in temperature (up to 38 degrees or higher), headache, lacrimation, swelling of nearby lymph nodes (especially with “internal” barley - see below), a feeling of a foreign body and other uncomfortable sensations. After a few days, the hordeolum is opened at the exit point of the fistula canal; purulent contents and particles of necrotic tissue are evacuated out, after which the symptoms are quickly reduced and soon disappear without a trace.
For an experienced ophthalmologist, establishing a diagnosis, as a rule, is not difficult: the clinical picture is well studied and varies slightly from case to case.
There are certain criteria for the differential diagnosis of typical barley:
- with meibomite, or “internal” barley (in particular, with a deeper location of the lesion, the purulent center opens not on the skin, but on the conjunctival surface of the eyelid);
- with inflammation of the lacrimal gland (one of the signs is the absence of a clear localization of inflammation - usually the entire eyelid swells, in severe cases blocking the main optical axis of the eye);
- with chalazion, or blockage of the meibomian gland - with this independent disease, the swollen area becomes practically painless in the days immediately following the manifestation, but can, unlike barley, remain unchanged for many months;
- with some others, incl. malignant, tumor neoplasms.
